Location: Fort Mohave, AZ 86426
Salary Range: $118,000 - $150,000 (USD)
Signing Bonus: $20,000 sign-on bonus and $5,000 relocation for a 3-year commitment
Reports To: Chief Nurse Officer
About the Position
Seeking a Director of Case Management to lead patient management strategies and improve quality, continuity, and cost-effectiveness of care. This senior-level role will provide expertise in assessment, planning, and patient-centered evaluation. The director will oversee case management, utilization review, and discharge planning functions to promote high-quality care and patient satisfaction. This is an on-site position based in Fort Mohave, AZ.
Key Responsibilities
- Oversee case management, utilization review, and discharge planning processes.
- Collaborate with medical staff and departments to achieve desired clinical outcomes and cost management.
- Lead interdisciplinary rounds to review treatment goals, optimize resource use, and identify post-hospital needs.
- Ensure compliance with medical standards and regulations.
- Act as a patient advocate, handle adverse occurrences, and provide staff education.
- Coordinate social services for patients and families to support healthcare benefits.
Requirements
- Bachelor’s degree in Nursing or higher (Required)
- BLS certification upon hire (Required)
- Arizona Registered Nurse or Compact State Registered Nurse license (Required)
- Minimum of 2 years as Director of Case Management in a critical care hospital (Required)
Preferred Qualifications
- CCM or ACM certification
- Master’s degree
- 5+ years of leadership experience in case management or critical care hospital management
