Job Description
We are seeking a full-time Social Worker for our new Behavioral Health Unit, opening in early Fall 2024. The Social Worker will provide direct, individualized care to patients, and support the development and implementation of patient care plans.
Key Responsibilities:
- Patient Assessment: Interview and assess patients, families, caregivers, and legal representatives to determine needs and priorities.
- Case Management: Participate in case reviews, consult with healthcare team members, and evaluate compliance with treatment plans.
- Discharge Planning: Assist with planning and processes, including arranging resources such as transportation, housing, and healthcare services.
- Supervision and Training: Supervise and train new staff, students, and interns.
- Screenings and Applications: Perform Medicaid screenings and assist with Safety Net applications.
Must-Haves:
- NV Licensed Clinical Social Worker (LCSW) license
- Master’s degree in Social Work
- Basic Life Support (BLS) certification from the American Heart Association
