Our PACE program provides comprehensive medical services to seniors, emphasizing holistic care. Collaborating with the interdisciplinary team, the Primary Care Provider conducts assessments, develops care plans, and ensures continuity of care for participants.

Essential Responsibilities:

Conduct in-person comprehensive assessments and continuous medical coverage.
Collaborate with physicians and IDT to develop and implement care plans.
Participate in discharge planning and nursing home care.
Oversee care coordination and review outcomes data for policy improvements.
Rotate night and weekend call duties.

Job Requirements:

Nurse Practitioner Certificate with Prescriptive Authority
NP State License
Minimum 2 years of experience with senior population
Knowledge of chronic care/geriatric best practices

Skills and Certifications:

Certificated as a Nurse Practitioner with Prescriptive Authority
Licensed in California
Geriatrics experience preferred.
2 years of NP experience
